What is a breast clip marker?

A breast clip, also called a marker, is a very small piece of body-safe metal placed in the breast to mark an exact spot. It comes in different shapes, such as a tiny coil, ribbon or clip. It is put in through a needle like the one used for a biopsy, so no stitches are needed.

The marker does nothing to the tissue. It is a landmark only. It shows on a mammogram and usually on a breast ultrasound, so a radiologist can tell later where a biopsy was taken.

When is a marker placed?

A marker is placed when the exact spot may need to be found again. Many biopsies need no marker at all. When one is used, it is usually placed at the end of a breast biopsy, but it can also be placed on a separate short visit, for example once a result is known.

  • After a biopsy of a small lesion, because a small lesion can be hard to find afterwards, or may be largely removed by the sampling itself.
  • Before treatment that may shrink the area, such as chemotherapy given before surgery, when a tumour can become hard to see on scans.
  • When more than one area is sampled, so that each can be told apart on later scans.
  • When surgery is planned, so that the surgeon or radiologist can find the area, sometimes with wire localisation shortly before the operation.
  • When an underarm gland has been sampled and needs to be found again at surgery.

Your doctor decides whether a marker is useful for you, based on the finding, the biopsy result and any treatment that may follow.

Is a marker safe to leave in the breast?

Yes, in general. Breast markers are designed to stay in the body permanently and are made of metals that the body tolerates well. Most people never notice theirs. It is not removed unless a surgeon takes it out as part of an operation on the marked area.

You should not feel it, and it does not limit your daily activities. It does not stop you having mammograms or other scans in future. On a mammogram it appears as a small bright spot, so tell any radiologist that you have one and keep the report that mentions it.

Placing a marker carries the same small risks as a biopsy: bruising, soreness, a little bleeding and, rarely, infection. Occasionally a marker lies a little away from the exact spot, or shifts slightly along the needle path as the breast is compressed. A mammogram after placement records where it lies, and the report notes any difference so the surgeon or radiologist can allow for it.

What happens when a marker is placed

  1. 1

    Review

The team reviews your biopsy plan or report, confirms that a marker is useful and tells you how to prepare. Your doctor decides whether you need one.

  2. 2

    Imaging and numbing

You lie on a couch. The doctor finds the area with ultrasound or mammography, cleans the skin and numbs it with local anaesthetic.

  3. 3

    Placing the marker

A needle is guided to the spot under imaging and the marker is released through it. The needle comes out and the marker stays.

  4. 4

    Check image

A mammogram may be taken afterwards to record where the marker lies. The report notes its position and type.

  5. 5

    Pressure and dressing

Pressure is held on the site for a few minutes and a dressing is applied. The team explains aftercare.

Preparation and aftercare

Before your visit

  • Eat and drink as usual, unless the team gives you other instructions.
  • Tell the team about blood thinners and any bleeding problem. Do not stop prescribed medicines unless your doctor tells you to.
  • Tell the team about allergies, including any allergy to metals such as nickel, and any chance of pregnancy.
  • A top that opens at the front or lifts easily is the simplest to wear.

Please bring

  • Your biopsy report or your doctor's plan
  • Earlier breast scans, with the images
  • Details of any earlier marker or breast surgery

After your visit

  • Keep the dressing clean and dry for the first day, or as advised.
  • Expect mild soreness or a small bruise, which settles over a few days. Ask the team before you take a painkiller.
  • Avoid heavy lifting and strenuous exercise for a day or two, or as advised.
  • Keep the report that records the marker with your other breast records, and show it at every future breast scan.

Warning signs after placing a marker

Soreness and a small bruise are expected. Contact the team or your doctor the same day, or go to the nearest emergency department if you cannot reach them, if you have:

  • Oozing or bleeding that will not stop with ten minutes of firm pressure.
  • Swelling that grows quickly, or a hard, tight, painful breast.
  • Fever, spreading redness or pus at the site.
  • Pain that gets worse after the first day.
  • Sudden breathlessness or chest pain, which needs urgent care.

Breast Clip / Marker Placement: common questions

Is the breast clip permanent?

Yes. Markers are designed to stay in place for life. The only time one is removed is when a surgeon takes it out along with the tissue during an operation on the marked area.

Can I feel the breast clip?

Almost everyone feels nothing once the soreness from the procedure has settled. The marker is far smaller than a lump and sits within the tissue. If you notice a new lump or pain at the site later, tell your doctor.

Does every breast biopsy need a clip?

No. A marker is placed only when the spot may need to be found again, for example because the lesion is small, several areas were sampled, or treatment may shrink the area. Your doctor and the radiologist decide.

Will the clip affect future mammograms or other scans?

It should not get in the way. On a mammogram it shows as a small bright spot and does not hide the breast tissue. Tell the person doing any scan that you have a marker, and keep the report that mentions it so that future doctors know it is expected.

What happens to the clip if I need surgery?

If surgery includes the marked area, the surgeon removes the clip with the tissue. When the area cannot be felt, the clip is often what the radiologist aims for when placing a wire shortly before the operation.

Can a clip be placed on a different day from the biopsy?

Yes. A marker can be placed on a separate visit, for example after a result is known or before treatment, when your doctor asks for it. The team reviews the referral and your scans first.

What if I have a metal allergy?

Tell the team before the procedure. Breast markers are made of metals chosen to be tolerated by the body, but if you know you react to a metal, your doctor can decide what is suitable for you.
