What is breast wire localisation?

Wire localisation is a short procedure that guides a surgeon to a part of the breast that neither of you can feel. A very thin wire is placed through the skin using imaging, so that its tip sits in or next to the area. The end of the wire stays outside the skin, and the surgeon follows it to the spot.

The wire is a marker, not a treatment. It does not remove or treat anything. It only shows the surgeon where to go.

The imaging that places the wire depends on how the area is seen. A breast ultrasound is used if the area shows clearly there, and mammography if it does not. Some surgical teams use other ways to mark an area, such as small implanted markers. Your surgeon decides which suits the operation.

When is a wire needed?

A wire is used when surgery is planned for an area that cannot be found by touch. Typical examples are a small abnormal area seen only on a scan, a cluster of tiny calcium specks on a mammogram, or a spot marked earlier with a clip marker.

Many people who need a wire have already had a breast biopsy, because surgery is usually planned once the nature of the area is known.

A wire is not needed for a lump the surgeon can feel. Whether you need one is your surgeon's decision, made from your scans and pathology report.

When is the wire placed, and who coordinates it?

The wire is placed shortly before the operation, often on the same day or the day before. The exact time depends on the surgeon's operating list. This is why the date is fixed with the surgical team first, and not by booking on your own.

Our team reviews your scans, your pathology report and the surgeon's plan. It then confirms whether the wire can be placed at the centre for your operation, who will place it and when to come. The operation takes place where your surgeon operates. The team will tell you when you book how the timing is coordinated.

Limits and risks

The wire helps the surgeon aim, but it cannot promise the result of the operation. Sometimes more tissue has to be removed if the first piece does not include the whole area. The removed tissue is checked afterwards, with imaging, to confirm that the marked area is in it.

Common effects are mild discomfort, bruising and the wire catching a little when you move. Less often there is bleeding or infection. The wire can occasionally shift, which is why you are asked to keep the area still and covered. Some people feel faint at the sight of a needle, so say so if that is you.

What happens on the day

  1. 1

    Plan with your surgeon

Your surgeon decides that a wire is needed and shares the plan, scans and biopsy report. The team reviews them and confirms the date, the time and how to prepare.

  2. 2

    Check and consent

On the day, the doctor confirms your details, the side and the area to be marked, and asks for your consent.

  3. 3

    Numbing and placing the wire

The skin is cleaned and numbed with local anaesthetic. A fine needle is guided to the area under imaging and the wire is passed through it. The needle is then withdrawn and the wire stays.

  4. 4

    Check images

A mammogram or ultrasound image is taken to check that the wire tip lies in the right place. The team shares the images with your surgeon.

  5. 5

    Covering the wire

The end of the wire is taped down and covered so it cannot be pulled. You then go on to the operation as the surgical team has planned.

Preparation and aftercare

Before your visit

  • Confirm the date and time with your surgeon first. The wire is placed only when the operation is arranged.
  • Follow the surgical team's fasting instructions for the operation. The team will tell you if anything else is needed for the wire.
  • Take your regular medicines unless told otherwise. Tell the team about blood thinners, bleeding problems and allergies, and do not stop any medicine unless your doctor tells you to.
  • Wear a loose top that opens at the front, so it can come off without pulling on the wire.
  • Bring someone to go with you to the hospital.

Please bring

  • Your surgeon's letter or plan
  • Biopsy and pathology reports
  • All earlier breast scans, with the images
  • Details of any clip marker placed earlier

After your visit

  • Keep the dressing in place and the wire still. Do not tug, press or adjust it.
  • Move the arm gently and avoid stretching or lifting it high.
  • Never cut or pull the wire. If it moves, bends or comes out, tell the team or your surgeon at once.
  • Go to the hospital at the time the surgical team has given you.
  • The surgeon removes the wire during the operation and explains the wound care afterwards.

Warning signs while the wire is in

Contact the team or your surgeon straight away, or go to the nearest emergency department, if:

  • The wire moves, bends or comes out, or feels as if it has gone deeper.
  • Bleeding soaks the dressing and does not stop with firm pressure.
  • The breast becomes swollen, tight or very painful.
  • You develop a fever, or the skin around the wire turns red or hot or leaks fluid.
  • You feel breathless, have chest pain or feel faint.

Breast Wire Localisation: common questions

Does the wire hurt?

Numbing is given before the wire goes in, so most people feel pressure rather than sharp pain. Afterwards the breast can feel sore, and you may feel the wire when you move. Tell the team if the discomfort is worse than you expected.

Is the wire placed on the day of surgery?

It is usually placed shortly before the operation, often the same day or the day before. The timing is fixed with the surgical team, who also tell you when to arrive at the hospital.

Does the wire stay in after the operation?

No. The surgeon removes the wire during the operation, together with the tissue around its tip. It is not meant to stay in the body.

Do all breast operations need a wire?

No. A wire is used only when the area to be removed cannot be felt. Lumps the surgeon can feel are usually removed without one, and your surgeon decides this from your scans.

What is the difference between a clip marker and a wire?

A clip is a tiny marker left in the breast, usually after a biopsy, and it stays there. A wire is temporary and is placed shortly before surgery. A clip can help the radiologist place the wire in the right spot.

What if my surgeon wants a different method to mark the area?

Some surgical teams prefer other markers, such as a small implant placed earlier. Follow your surgeon's plan. If you already have a marker in your breast, tell us when you book so the team can review it.
