This package is for people whose doctor suspects a problem with calcium control. The parathyroid glands are four tiny glands behind the thyroid. They release PTH to keep blood calcium steady.

• A high or low calcium result on a routine blood report.
• An abnormal PTH that your doctor wants to follow up.
• Kidney stones that keep coming back, when calcium is among the causes your doctor wants to rule out.
• Bone pain, fractures or thinning bones with no clear reason.
• Thirst, a lot of urine, constipation, low mood or long-standing tiredness, together with a calcium result outside the range. Without that result, these symptoms have many other causes.

If your symptoms are mainly weight change, palpitations or a neck swelling, you probably need the thyroid package. If your main concern is bone strength, look at the osteoporotic bone profile, which adds phosphorus, alkaline phosphatase and a kidney ultrasound.

These tests are read together, because PTH and calcium push each other up and down. PTH rises when calcium falls, and it should fall when calcium rises.

• Serum calcium is the amount of calcium in your blood. It is the starting point, because healthy parathyroid glands keep it within a narrow range.
• Serum PTH is the hormone itself. A raised calcium together with a PTH that is high, or not low as expected, is the pattern that makes doctors think of an overactive gland.
• Vitamin D3 is the vitamin that lets the gut take up calcium from food. Low vitamin D can push PTH up even when the glands are healthy, so it has to be known before PTH can be read.
• T3, T4 and TSH check the thyroid, which lies in front of the parathyroid glands. An overactive thyroid can raise calcium a little, so doctors often check it too.
• The parathyroid ultrasound looks at the neck behind the thyroid for an enlarged gland. It uses sound waves, not radiation.

A normal parathyroid gland is no bigger than a grain of rice and is hard to see on ultrasound. An enlarged one can sometimes show as a small oval nodule behind the thyroid.

A scan that sees nothing does not mean the glands are normal. Ultrasound misses some glands, especially those low in the neck or behind the breastbone. If the blood tests and the scan do not agree, your doctor may suggest other imaging. We do not have a CT or MRI scanner, so your doctor will advise where to have it done, and the team can review the images with you.

One result rarely settles the question. Calcium, PTH, vitamin D and kidney function affect each other, and medicines such as water tablets and supplements can shift them. Your doctor, often an endocrinologist, reads them together and may repeat some tests.
