What kind of breast pain do you have?

Doctors group breast pain into three patterns, because the pattern points to the cause.

  • Cyclical pain builds before a period, affects both breasts, feels heavy or tender, and eases once the period starts.
  • Non-cyclical pain does not follow your periods. It may be constant, may sit in one area, and can burn or ache.
  • Chest-wall pain feels as if it is in the breast but comes from the muscles or ribs behind it. It often hurts more when you move or press on the spot.

What causes breast pain?

Most breast pain comes from normal hormone changes. Oestrogen and progesterone rise and fall through the month, and the breast tissue responds. Pain can also appear in pregnancy, during breastfeeding and with some medicines, including hormonal treatments.

Other causes include a breast cyst, which gives a tender swelling in one spot. A breast infection (mastitis) brings redness, heat and fever. An injury, an old scar, a bra that fits poorly or a strained chest-wall muscle can also hurt. In many people no cause is found, and that is not a worry in itself.

Breast pain on its own is rarely caused by cancer. Most breast cancers do not cause pain at first. Pain together with a new lump or another change is the reason a doctor should see you, as described on the breast lump evaluation page.

When does breast pain need a scan?

Many people with breast pain do not need a scan. If the pain follows your cycle, there is no lump and the examination is normal, a doctor can often reassure you without imaging. A scan is useful in these situations:

  • Pain in one area that does not settle over a couple of menstrual cycles.
  • A new lump, thickening or firm area along with the pain.
  • Nipple discharge, a nipple change or a skin change such as dimpling or redness.
  • Swelling or tenderness in the underarm.
  • An earlier abnormal scan, or a strong family history of breast cancer.

The usual scan is a breast ultrasound, which examines a tender spot closely. A mammogram is added in older women, or when the doctor advises it. Neither scan measures pain. They look for a cause such as a cyst or a lump, and a normal result is reassuring.

What can ease breast pain?

Simple steps often help, and your doctor can suggest more. A well-fitting, supportive bra, worn day and night if that is more comfortable, helps many people. A pain diary kept over two or three cycles shows whether the pain follows your periods.

Your doctor may also review your medicines. Do not stop any prescribed medicine on your own. If you want to take a painkiller, ask your doctor which one suits you.

What happens at your visit

  1. 1

    Booking

Message or call us with your doctor's note and any earlier breast scans. Tell the team where the pain is, how long you have had it and whether it follows your periods.

  2. 2

    History and examination

You are asked where the pain is, when it comes, how it relates to your periods and what medicines you take. The breasts and underarms are then examined.

  3. 3

    Scan if needed

A breast ultrasound is done over the painful spot. A mammogram is added when your age or findings call for it.

  4. 4

    Findings and report

The radiologist reviews the images and prepares the report. The team will tell you when and how you will get it.

  5. 5

    Plan with your doctor

Take the report to your doctor to agree on reassurance, simple measures or follow-up.

Preparation and aftercare

Before your visit

  • Mark in a diary where it hurts and on which days, for a month if you can.
  • List your medicines, including hormonal tablets or injections.
  • Say if you are pregnant, may be pregnant or are breastfeeding.
  • Wear a supportive bra and a two-piece outfit so the breasts are easy to reach.

Please bring

  • Your doctor's note
  • Earlier mammograms or ultrasound images and reports
  • Your pain diary, if you kept one

After your visit

  • There is no recovery time after the scans, and most people go straight home.
  • Show the report to your doctor even if it is normal, and talk through what your pain pattern suggests.
  • Note whether the pain changes over the next cycles, and tell your doctor if it gets worse.

When breast pain needs a doctor soon

Arrange to be seen soon, rather than waiting for a routine booking, if:

  • The pain comes with a new lump, nipple discharge, a nipple turning inward or a change in the skin.
  • The breast is red, hot, swollen and tender, or you have a fever. This can be an infection that needs treatment.
  • The pain stays in one spot and has not eased after a few weeks.
  • The pain is severe, wakes you at night or stops you doing daily tasks.
  • You have chest pain with breathlessness or sweating, or pain spreading to the arm or jaw. This may be a heart problem and needs emergency care.

Breast Pain Evaluation: common questions

Can breast pain be a sign of cancer?

Rarely. Pain alone is not usually a symptom of breast cancer, and most breast cancers do not hurt at first. Pain together with a lump, discharge or skin change is different and should be checked. An examination and scan can settle the question.

Why do my breasts hurt before my period?

Hormone levels change through the cycle and the breast tissue swells slightly, so both breasts may feel full, heavy or tender in the week or two before a period. It eases when the bleeding starts. This is normal and needs no scan unless you also notice a lump or the pain stays in one place.

Which scan is done for breast pain, ultrasound or mammogram?

For one tender spot, ultrasound usually comes first. A mammogram is added in older women, or when the doctor advises it. Your doctor decides, and the team confirms the plan when you book.

Can a bra cause breast pain?

A poorly fitting bra can cause or worsen pain, especially with heavy breasts, during exercise or at night. A supportive, well-fitting bra is a simple first step. If the pain continues after a change, see a doctor.

Is breast pain normal in pregnancy or while breastfeeding?

Yes, breasts are often tender in early pregnancy because of hormone changes. During breastfeeding, pain can come from a poor latch, a blocked duct or an infection. Redness, heat or fever need prompt attention. Ultrasound uses no radiation and is the usual scan in both situations.

How long does breast pain last?

Cyclical pain eases after the period and may return each month. Pain from other causes can last longer. If pain has gone on for many weeks or keeps coming back in the same spot, have it examined.
